  • Laboratory diagnosis plays an important role in disease and outbreak management. Fast and accurate laboratory diagnosis of a specific viral infection of interest contributes to prompt public health surveillance, prevention, and control measures
  • According to the Johns Hopkins Center for Health Security, diagnostic testing for COVID-19 is mainly undertaken through the use of two approaches:
    1.whole genome sequencing and real-time reverse transcriptase PCR (rRT-PCR). Genome sequencing was used primarily early in the outbreak to facilitate initial identification of the virus. Current diagnostic testing for COVID-19 is mostly done using rRT-PCR.
    2.Prior to the Chinese authorities’ decision on 12 February to expand the diagnosis criteria for COVID-19 to include clinicallydiagnosed cases, nucleic acid testing (NAT) or RNA tests through the rRT-PCR process was the primary diagnostic tool.
  • The Department of Health Research (DHR) / Indian Council of Medical Research (ICMR) initiated establishment of a network of public health laboratories to enhance capacity for diagnosis and detection of viruses of public health importance in India
  • Following the increase in load of screening samples from suspected cases with symptoms and travel history to the affected countries, DHR/ICMR has commissioned 51 laboratories for COVID-19 testing in India  In addition, NCDC Delhi is also engaged in testing. (Total = 57 laboratories) All these laboratories have an operational Real-Time PCR platform
  • The protocols for diagnostics using RT-PCR published on WHO’s webpage is for guidance and not an exhaustive list. Various institutions and governments have chosen to develop their own protocols that might not be publicly available or published by WHO on their webpage.
  • The Indian Council of Medical Research (ICMR) has approved 176 labs, including 47 private labs, to conduct the tests
  • The first test kit that received FDA approval, the Cobas SARS CoV-2 kit from Roche, is also approved in India. The second, the TaqPath COVID-19 Combo Kit from Thermo Fisher, is in the process of getting validated by the ICMR labs.
  • As long as proper certification and training are in place, any accredited lab using the WHO/CDC suggested protocols can yield satisfactory results.
